PT Clinician

The Part-Time Clinician will provide oversight and guidance to case managers by reviewing case notes, ensuring documentation accuracy, and training staff on best practices for progress tracking. This role is critical in maintaining compliance, improving service delivery, and supporting case managers in effectively documenting client progress.

Roles & Responsibilities:

  1. Case Note Review & Quality Assurance

    • Review case notes submitted by case managers to ensure accuracy, completeness, and compliance with organizational and regulatory standards.

    • Provide constructive feedback to case managers regarding documentation improvements.

    • Ensure that progress is being documented appropriately, with clear and measurable goals.

  2. Training & Development

    • Educate case managers on best practices for case note documentation, including proper language, structure, and compliance requirements.

    • Conduct training sessions or one-on-one coaching as needed to enhance the quality of case notes.

    • Guide case managers on accurately recording client progress, interventions, and outcomes.

  3. Compliance & Risk Management

    • Ensure that all case documentation aligns with CalAIM program requirements, HIPAA regulations, and other relevant standards.

    • Identify gaps or inconsistencies in documentation and work with case managers to resolve them.

    • Maintain confidentiality and ethical standards in reviewing case notes.

  4. Collaboration & Support

    • Work closely with case managers to provide ongoing support and guidance in case documentation.

    • Partner with supervisors and leadership to identify trends in documentation issues and recommend solutions.

    • Assist in refining documentation policies and procedures to enhance efficiency and effectiveness.

  5. Reporting & Continuous Improvement

    • Provide regular updates to supervisors on case note quality and areas for improvement.

    • Recommend process improvements to streamline documentation practices and enhance case management services.

    • Stay informed on industry best practices and evolving documentation standards to ensure continuous compliance.
