Case Manager (housing assistant)

Position Overview:
We are seeking a compassionate and detail-oriented Case Manager CalAIM Housing Support to assist clients in navigating housing services and securing stable, permanent housing. This role is ideal for someone passionate about helping individuals experiencing housing instability and addressing complex social and medical needs.
Key Responsibilities:
Client Assessment: Conduct comprehensive assessments to determine clients' housing needs and eligibility for CalAIM services.
Housing Navigation: Assist clients in identifying, securing, and maintaining stable housing by working closely with landlords, property managers, and housing authorities.
Service Coordination: Connect clients with essential services, including mental health care, substance use treatment, and other social support programs.
Care Planning: Develop, implement, and manage individualized care plans that integrate health and housing goals.
Monitoring and Documentation: Track client progress, maintain detailed service records, and adjust care plans as needed.
Collaboration: Partner with healthcare providers, social workers, and community organizations to deliver comprehensive and coordinated care.
Advocacy: Support clients in accessing housing and healthcare services while advocating for their rights and needs.
Qualifications:
Education: Bachelor's degree in social work, psychology, public health, or a related field (Master’s degree preferred).
Experience: Previous experience in case management, housing support, or a related field is required.
Skills: Strong communication, organizational, and problem-solving abilities; familiarity with local housing resources and social service programs.
Technical Proficiency: Ability to use case management software and electronic documentation tools.
Additional Requirements: Valid driver’s license and reliable transportation for community-based work.
